Meet the Founder

Hazel Jacobs, LCSW, LISW-CP, MSW

With 22 years of clinical experience and a Masters in Social Work from Norfolk State University, Hazel founded Coalesce Family Services with a clear mission: to empower individuals, families, and communities to become whole again.

Licensed to serve clients across all of South Carolina and Virginia, Hazel specializes in CBT and mindfulness, providing a nurturing environment for tackling mental health challenges. Her approach, customized for each client, emphasizes resilience, emotional control, and communication.

As a board-certified supervisor in both Virginia and South Carolina, she also empowers the next generation of Licensed Clinical Social Workers through personalized clinical supervision.
