Hazel Jacobs, LCSW, LISW-CP, is a compassionate, experienced therapist dedicated to helping individuals and families overcome challenges, find clarity, and build resilience. With both her Bachelor’s and Master’s degrees in Social Work from Norfolk State University, Hazel has been passionately committed to serving others throughout her career.
Before founding Coalesce Family Services, Hazel provided clinical support to service members, veterans, and their families across the U.S. and internationally in East Asia and Europe. Her extensive experience with diverse populations has deeply informed her integrative approach to therapy.
Hazel believes therapy should be tailored to each person's unique story, combining evidence-based treatments such as cognitive-behavioral therapy (CBT), mindfulness, and trauma-informed care, with holistic strategies to nurture the mind, body, and spirit. She places connection and rapport at the heart of her practice, creating a safe, judgment-free space where genuine healing and growth can occur.
Hazel specializes in supporting individuals dealing with anxiety, depression, trauma recovery, family relationships, and transitions unique to military life. Her greatest fulfillment comes from empowering clients to achieve lasting, positive change in their lives.
